- First trimester nausea often limits weight gain. Gaining little or even losing a few pounds in weeks 1–13 is common and generally not a concern.
- Focus on nutrient density, not just calories. Folate, iron, calcium, and DHA are especially important during pregnancy.

Why does pre-pregnancy BMI affect the recommended gain?
The Institute of Medicine (IOM) guidelines are designed to optimize outcomes for both mother and baby. Underweight women need more gain to support fetal growth. Overweight and obese women need less gain to reduce risks of complications like gestational diabetes, preeclampsia, and cesarean delivery.
When does most pregnancy weight gain occur?
Typically 1–5 lbs in the first trimester, then roughly 1 lb per week for normal-weight women in the second and third trimesters. The fetus, placenta, amniotic fluid, increased blood volume, and breast tissue all contribute to the gain.
What makes up pregnancy weight gain?

Is it safe to lose weight during pregnancy?
Intentional weight loss during pregnancy is not recommended and can deprive the fetus of needed nutrients. Even obese women should gain some weight. If you are gaining much faster than recommended, speak with your healthcare provider about healthy habits rather than restricting intake.
